We require proof that you are the parent or guardian when performing a piercing on a minor, proof of this can be established through two government issued ID's, a original Birth certificate or a notarized copy of it.
ID TYPES: State issued ID cards or drivers license, Passport, visa, birth certificate.

Are you not the maternal or paternal parent but are the guardian? we will need to see court or notarized documents stating custody. Without that at least one legal guardian needs to be present.
